BikeMaps.org is a crowd sourced map of cycling collision data worldwide. Incidents of collisions, near misses, hazards and thefts can be entered via the web site or through the free Android or Apple apps. The data collected is available to all. If you choose to register and log in, it will even alert you to recent incidents mapped in your area.
Update October 2021: Karen Labaree at BikeMaps.org advises that these apps are now out of date and in need of overhaul. She requests that you make these reports through the BikeMaps website instead.

A report from the Traffic Injury Research Foundation evaluated this tool and found a number of benefits that include municipalities who use the data to plan cycling infrastructure and perform infrastructure upgrades identified by hotspots.
