A car enters a 6-lane deep intersection on a green light with intentions to go straight through but the car in front of him stops mid-intersection and turns on it's left turn signal, causing the car behind to stop and wait. The light turns yellow and then red before the vehicle turning left is able to proceed. The car behind now must exit the intersection but the perpendicular lanes of traffic now have a green light.
Is the onus on the driver of the car in the intersection (now with the red light) to proceed with caution or is does the responsibility lie with the drivers facing the green light to enter the intersection only when it is clear?
I've more or less answered your question in my article titled Stuck in an Intersection. However, the driver who is stuck in the intersection must also exercise due care, especially since there are bound to be drivers who think that the green light tells them to go regardless...

This is a perfect of example of what the horn was intended for. The horn was intended to draw attention to you. It would be wise to give one or 2 blasts of the horn to alert others you are still there so that they dont drive into you. But do get out of the intersection. Legally all drivers are suppose to allow you to clear the intersection.
